Welcome to Wentworth 1

Primary Care Network

Wentworth 1 Primary Care Network (PCN) was developed in 2019. PCN’s were created to transform the way people access healthcare and support from within GP practices.

Wentworth 1 PCN consists of seven GP practices in Rotherham. We offer a wide range of services to meet the needs of our patients and our aim is to provide our patients with the best possible care from our GP Practices.

Wentworth 1 PCN is responsible for the provision of specialist support staff who work across our seven practices. This can include staff such as Clinical Pharmacists, First Contact Physiotherapists, Health and Wellbeing coaches, Physician Associates and Social Prescribers. You can read more about these additional roles and how you can access them on our people you might meet page.

What is a PCN?

A primary care network is a group of health care professionals who work together to provide comprehensive, coordinated care to patients.

Primary care networks can include doctors, nurses, pharmacists, and other health professionals. They can help to improve the quality of care that patients receive. By coordinating services and collaborating on treatment plans, primary care networks can ensure that patients receive the best possible care.
